Medicare stars (nursing homes) come from CMS Care Compare: an overall 1–5 rating built from health inspections, staffing and quality measures.
Florida stars (nursing homes) come from the State of Florida's Nursing Home Guide, based on the last three years of state inspections.
Inspection record (assisted living) summarizes the State's public record for the past five years: substantiated complaints, Class I–II deficiencies (the most serious citation classes), sanctions and fines.
Google reviews open each facility's live Google listing, so you always see the current star rating and newest reviews.
